Terms and Conditions of Use

By agreeing to the following terms and conditions, I acknowledge that I am requesting access to portions of my personal health information and the ability to communicate within the integrated academic health system of Northwestern Memorial HealthCare, which includes my healthcare team ("Northwestern Medicine") concerning my health information via the Internet using an electronic application called MyChart.

I hereby expressly authorize Northwestern Medicine to disclose my identifying health information to my designated MyChart account for the purposes described herein. My continued use of this site indicates my agreement to abide by these terms and conditions.

I understand that MyChart should never be used for urgent matters. If I am experiencing a life-threatening illness or injury, I will go to the nearest hospital emergency department and/or dial 9-1-1 for immediate attention. I understand that messages can only be sent to Northwestern Medicine providers. If I send a message to my provider through MyChart, I understand that the message are automatically routed to a nurse. The nurse may respond to me directly, or forward my message on to the provider. Because the anticipated turnaround time for a response to electronic messages may be up to two business days during posted business hours, I will contact my Northwestern provider by phone if I have a matter that requires attention today and cannot wait. If I do not get a response in 2 business days, I understand it is my responsibility to contact the Northwestern Medicine provider to obtain resolution.

I understand that all health-related information provided by this website, other than my personal health information, is intended to be for general use and should not be used as a substitute for a personal visit to a health care professional. Since the information I obtain from the site may be appropriate for general use but inappropriate for my own situation and could be misinterpreted, Northwestern Medicine shall not be responsible for how I use the information obtained. I am responsible for all information that I submit to or through MyChart.

I understand that my Northwestern Medicine care team may send messages to me via MyChart. By entering my valid and functioning email address, I have enabled Northwestern Medicine to notify me of messages sent to my MyChart inbox. I will keep my email address updated. I understand that if I elect to receive messages through MyChart it means that I will not necessarily receive notifications by letter or telephone. These messages may contain information important to my health and medical care; therefore it is my responsibility to monitor these messages. Northwestern Medicine, its employees, and health professionals under contract to Northwestern Medicine shall not be liable for any loss, injury, or claims of any kinds resulting from MyChart messages I fail to read in a timely manner.

I agree that all communication through MyChart will be in regard to my own health condition or to the health condition of an individual whose information I have obtained proxy access for and that the contents of any message may be stored in my or the individual's permanent medical record. I understand that asking for advice or service on behalf of another person (other than an individual for which I have obtained proxy access) could be potentially harmful and is a violation of the MyChart terms of use. Northwestern Medicine does not assume responsibility for and disclaims any and all liability in connection with health information or services on this website for anyone other than MyChart enrollees. I will not use or exploit any portion of MyChart to provide any commercial services to third parties.

After I have been provided with a one-time-use access code, I will access MyChart and through prompts create my unique user identification (UID) code and password. I understand my UID and password are unique codes that identify me in the MyChart application and allow me to access my personal medical information. Inquiries and entries I make via MyChart will be logged with my identity. If my access code is expired or doesn't work, I can request a new one at any time through our online enrollment process and have it sent to me via email, text message or letter. I understand that it is extremely important that I keep my UID and password completely confidential. If at any time I believe the confidentiality of my password has been compromised, I will change it by going to the Password link on the MyChart site. I understand that Northwestern Medicine HealthCare accepts no responsibility for and disclaims any and all liability or consequential damages arising from a breach of medical record confidentiality resulting from my sharing or losing my password. If Northwestern Medicine discovers that I have inappropriately shared my password with another person, or that I have misused or abused MyChart access privileges in any way, Northwestern Medicine may discontinue my participation in MyChart without prior notice.

I understand that members of Northwestern Medicine have the discretion to select the information that will appear in the MyChart application. I understand that the MyChart information is not a complete representation of the medical information Northwestern Medicine may include within my full medical record, and in the event another treating provider requires a complete record of medical information obtained or maintained by Northwestern Medicine, I acknowledge I must request a full copy of the medical record from Northwestern Medicine.

Northwestern Medicine reserves the right to revoke my access to MyChart at any time for any reason.

Information Security

User names and passwords provide two layers of authentication and are stored in a secured database that is isolated from the Internet. As a MyChart user, my role in maintaining the security of my medical information is: 1) changing my password on a regular basis, and 2) keeping my UID and password confidential. I agree to notify Northwestern Medicine immediately in the event that (i) my password is lost or stolen, or (ii) I become aware of any unauthorized use of my password or of any other breach of security that might affect MyChart. Northwestern Medicine is not responsible for any loss or damage arising from my failure to comply with this provision.

All communications between me and Northwestern Medicine via MyChart are secure. Northwestern Medicine will only use MyChart for computer-based electronic medical communication. While I may receive Internet email messages notifying me of new messages in my MyChart inbox, these emails will not contain any confidential medical information.

The information I view in MyChart is protected by using Secure Socket Layer technology. I can tell that I am viewing a secure website by verifying the address starts with "https://" rather than "http://". Whenever I see "https://", unauthorized individuals cannot read my data. HTTPS (Hypertext Transfer Protocol over Secure Socket Layer or HTTP over SSL) is a web protocol that encrypts and decrypts user page requests as well as the pages that are returned by the web server.

Minimum Browser Requirements

I understand that internet access is required to access MyChart.

Verification of Identity

I understand that my enrollment is contingent upon verification of my identity by a Northwestern Memorial HealthCare associate or provider.

Minor Eligibility

To request a personal account, an individual must be at least 12 years old. Parents or guardians of minor children may request access to the minor child's account by following the terms outlined in the "Proxy Access" section of this Terms and Conditions statement. Minor children between the ages of 12 and 17 (inclusive) will be eligible to participate in the full spectrum of MyChart services with provider approval. Under State and Federal law, there are certain types of medical information that the parent or guardian of a minor patient age 12 or older may not view without consent of the minor patient. Because of these requirements, we cannot offer the full spectrum of services to parents of minor patients in this age category unless the minor approves Proxy access to the parents. Thus, when a minor patient reaches age 12, restrictions will be placed on MyChart access for parents until he/she reaches age 18.

Proxy Access (also known as Personal Representative)

Individuals age 18 or older may request proxy access to another individual's MyChart account. Emancipated minors may be given proxy access on a case-by-case basis. The information available in MyChart will be left to the discretion of Northwestern Medicine, and Northwestern Medicine reserves the sole and absolute discretion to decide whether proxy access will be granted.

If the proxy's legal relationship with the patient changes, the proxy must inform their MyChart Representative immediately at MyChart@nm.org or 1.855.HLP.MYNM (855.457.6966). Northwestern Medicine reserves the right to revoke proxy access at any time for any reason.

Northwestern Medicine does not endorse and has not verified the accuracy of the information in/on these web sites, and I should not rely on any of the information found on the web sites for purposes of treatment or diagnosis.

Privacy

I understand my privacy is of utmost importance to Northwestern Medicine and I may review its Notice of Privacy Practices for a thorough description of how my personal health information is collected, utilized, and protected.

Disclaimer

MyChart may not be available to users all the time due to system failures, back-up procedures, maintenance, or other causes beyond the control of Northwestern Medicine. Access is provided on an as-is, as-available basis, and Northwestern Medicine does not guarantee users will be able to access MyChart at any particular time. MyChart is not designed to make diagnoses or to help users make diagnoses. Only my physician can properly diagnose and treat my illness or injury. MyChart is not a complete medical record, and should not be used in any situation that requires a full and complete medical record.

My MyChart does not include all information from my permanent medical record. For example, My MyChart does not include a physician's notes from my visit. Changes

Northwestern Medicine reserves the right to add to or change MyChart or to cease offering MyChart at any time without notice to me. Northwestern Medicine shall not be liable to me or to any third party for any modification, suspension or discontinuance of MyChart. Northwestern Medicine reserves the right to refuse to offer MyChart to anyone at any time without notice.

If I agree to the foregoing Terms and Conditions, I will indicate my agreement by clicking the box labeled "Accept." If I do not agree to the Terms and Conditions I will click "Decline" and I may not use MyChart.
